Required documents to accompany the application include:

  • A completed work permit application form ;
  • A statement regarding criminal record, signed by the employer, as required by the law;
  • A copy of the saved pages of the foreigner’s passport;
  • Confirmation of the application fee of PLN 200 for each application submitted;
  • Contract for the provision of the service in Poland;
  • Original power of attorney for the person authorized to represent the foreign employer in Poland;
  • Document confirming the legal status, form or nature of the business conducted by the foreign employer.

Our employees can represent your company before state institutions, such as Social Security, the Tax Office, as well as others, according to individual arrangements.
